What made me black out and wake up somewhere else?
I drank something in Fallout 76 that darkened my screen, then I blacked out and woke up far across the map. What was it?

That sounds like Nukashine. Drinking it causes a blackout and sends you to a random location, with a screen effect as it happens. If you want to repeat it, look for Nukashine or make some using its recipe.
