I’ve noticed that when I see a green penetration indicator, I often end up with ricochets or non-penetrations, and it doesn’t make much sense to me. I suspect the indicator doesn’t factor in things like range or the enemy’s armor upgrades. Can someone explain how this works?

Yeah, just because the indicator is green doesn’t mean it’s a guaranteed hit. If there’s dispersion, your shot might not hit the armor squarely, which can lead to it bouncing off. That’s why mods that show battle hits can really help visualize how this works, since they take client-side hits into account.
The green pen indicator usually means you have over a 75% chance to penetrate your target. However, there’s still a chance that you could get a low roll and end up bouncing, especially due to dispersion affecting where your shot actually lands.
Just keep in mind that the green indicator is only about where you’re aiming. If your shot doesn’t hit precisely where the indicator is, that could lead to non-penetration. The shot can deviate a bit due to dispersion, which is something to watch out for.
Actually, the green indicator suggests that even with a -12.5% pen roll, you have about a 93.9% chance to pen. So theoretically, you could still bounce, but it’s pretty unlikely with a green status.
