Has anyone else noticed that the game often fails to properly track achievements and challenges? For example, there’s an achievement for repairing 76 ranged weapons, but my game only says I’ve repaired 9, even though I’ve spent around 200 hours mostly using ranged weapon builds. Also, the achievements for reaching levels 25, 50, and 100 haven’t unlocked for me, despite being almost level 200. Is this a common issue, or is something just messed up in my game? I’ve noticed more and more milestones that don’t seem to update correctly.

Definitely not just you. I’m level 700 and still missing one disease achievement even though I’ve gone out of my way several times to catch all diseases. The tracker just doesn’t seem reliable sometimes.
I noticed some achievements, like modding shotguns, don’t register even though I know I’ve done it because I have modded shotguns in my stash. Also, it seems like the game only counts active playtime, maybe ignoring time spent in loading screens or menus. That might explain why my total recorded playtime is less than what I actually spent.
Loading screens feel like part of the game on consoles though! On Xbox One, those wait times are crazy, so it really skews the playtime tracking. Definitely a Bethesda classic issue.

Haha yeah, I had that too! It said I didn’t have sludge lung for the longest time even when I was actively infected. Feels like a glitch for sure.
