He’s running around doing everything just fine with his three fingers; it really stands out to me. So, I’m curious, what’s the reasoning behind this choice?

Well, both RE7 and RE8 kind of revel in these over-the-top injury moments, almost like a twisted love letter to hand models! It’s part of that whole horror vibe, I guess.
I think they wanted to highlight that we only get a glimpse of Ethan, much like how other characters in the series have injuries that are visible, like Leon and Jill. It’s almost like a visual cue for how tough they are, you know?
