Got the serum from the Institute. If I give it to Virgil, whats the reward? What happens if I keep it?

Curing Virgil just gives good karma, you see him as a human later. Keeping it has no real benefit. Just do what you feel like.
Yeah, pretty much nailed it. The whole Virgil serum decision is mainly just about story and role-playing, rather than your personal in-game stats or rewards. Here’s a bit more to chew on:
Morality and Role-Playing: Helping Virgil by giving him the serum can make you feel like you’re doing the right thing. It goes along with a “good guy” playthrough. Depending on how you’re playing your character, this could align with your role-playing choices.
No Direct Benefits: Honestly, you’re not gonna see any caps rain down on you or some epic weapon pop up in your inventory for helping him out. If you keep the serum for yourself, there’s literally no advantage gameplay-wise. So, it’s not really a decision with mechanical consequences.
In-Game Changes: If you do give him the serum, and you visit him later, you’ll find that he’s back to being human. It’s a pretty neat change and makes your world feel a bit more responsive to your actions.
Quests and NPC Reactions: Not giving the serum doesn’t lock you out of any quests or make other characters mad at you or anything. It’s just about that one interaction with Virgil and how his story concludes.
Overall, think about what kind of character you’re playing. Do you wanna be a hero helping out the downtrodden, or more of a survivor looking out just for yourself? Whatever you choose, it’s good role-play content.
If you give Virgil the serum, after a few in-game days he turns back into a human. You can visit his cave and talk to him for some extra XP and a bit of a happier ending for him. Just make sure you have it with you when you chat with him.
Doesn’t really matter. If you give it to Virgil he turns human but you don’t get anything special. No consequences if you keep it but giving it to him makes the world a bit better maybe.
