I have an idea for a small feature regarding ghouls in Fallout: what if eating and drinking could slightly boost their sanity meter, maybe up to 30%? Additionally, sleep could prevent the sanity from decreasing. In Fallout 1, we see that dehydration can lead to madness for necrotic folks, so it’s reasonable to think that even just a bit of food or water could help keep ghouls sane. There are examples throughout the series, like Coffin Willie in Fallout 2, who survived underground without sustenance for a long time, and Dean Domino in New Vegas, who mentions eating and drinking out of habit rather than necessity. It seems like adding this mechanic could help players understand the ghoul experience better, while also acknowledging that they still require food and water as shown in Fallout 4. What do you think?

Honestly, I’m on board with OP’s suggestion! It would make sense for ghouls who eat, like my buddy who’s a chem junkie, struggling to find food. Giving them slight benefits from food could really enhance gameplay. It’s a bit rough when he’s constantly looking for chems instead.
I’m not sure about this change. It could really screw over players who enjoy playing as feral ghouls. They rely on that cannibal perk to manage their glow meter. Wouldn’t want to make it harder for them, right?

I don’t think this is a great idea. They’ve already simplified the hunger and hydration system, so bringing it back in any form for ghouls seems unlikely. Plus, this game is about survival in harsh conditions, shouldn’t ghouls lean into that struggle?
