I’m currently running mythic+ dungeons as a blood dk, but I’m starting to feel like my gear is holding me back. I’m doing +8 keys with an item level of around 645. When should I start worrying about my gear? What item level should I aim for before pushing into +10 dungeons?

I’d say aiming for 660+ is a safe bet for +10s. It gives you a bit of a buffer. I remember when I was healing in 640 gear, I managed to push 10s, but it really depends on your group’s skill level as well. If your group is solid, you can make it work often regardless of gear.
I was in an M6+ with a tank who had a flat ilvl of 600, and they didn’t die the entire run! Gear can be important, but sometimes it’s all about player skill. Just know that you might find yourself struggling with lower skilled groups, regardless of your gear.
+8 with a 645 ilvl is actually impressive! You’ll start receiving gilded crests from those runs, which means item upgrades are on the way. Just keep pushing and you’ll hit 670 in no time!
645 is decent, especially if you know your class well. But since +8 dungeons drop gear around 658, it might be wise to bump your ilvl a bit more. Consider crafting items or running some 5-mask visions for extra upgrades, and maybe tackle some normal or heroic raids to help gear up before pushing higher keys.
Gear isn’t a huge problem in mythic+, since the mobs scale with your ilvl. The lower your ilvl, the more you’ll feel the challenges at higher keys. But if you strategize well, you could still succeed even if your gear isn’t top tier.
You can get 655 drops from keys 10 and up, but it’s not impossible to do these with lower gear if you play well. I started healing 8s with 640 and ended up doing a bunch of 12s later on. It’s all about how well you communicate and coordinate with your team.
