There is no way to equip it and swing like you can do in games like Uncharted. The way I got around this was to just jump. Press X and if you are quick enough, you will be able to press R2 and be able to hook onto the grapple point without needing to be in the air since you are technically in the air. It is a stupid workaround but it has worked for me.
