I haven’t played Kingdom Come: Deliverance since its original launch and never had any issues with screen tearing during my 140-hour playthrough. However, after trying out the new hardcore mode recently, I’ve noticed a lot of screen tearing both during gameplay and in cutscenes. Has anyone else encountered this problem or found a fix?

Yeah, I noticed the same thing. Turns out my monitor’s refresh rate got changed by Windows after an update which made it worse. I went into Display settings > Advanced display settings and bumped the refresh rate back up from 60Hz to what it should be, and that helped a lot.
Check if you’re running the latest Windows 11 24H2 update or using NVIDIA’s optimization through their control panel. Both caused me bad screen tearing. I fixed it by rolling back the Windows update and disabling NVIDIA optimization for KCD2.
I don’t use the NVIDIA app, but I’ll definitely look into the Windows update angle. The game has high FPS for me, but it still feels choppy with tearing and micro stutters, which I never experienced before.
Have you tried toggling VSync? Sometimes it helps with tearing issues.
I actually tested with VSync both on and off, but the tearing still happens either way.
