Starting Out at Brookhaven Hospital

Examine the board on the right to find a ‘Map of Hospital.’
Head forward and into the door right in front of you (Reception Office). Save your game.
Pick up the ‘Health Drink’ off the table beside the East door, then go through it.
Examine the document beside the typewriter to receive the ‘Purple Bull Key.’

Move to the 2nd Floor

Climb the stairs and open the door to 2F.
Defeat enemies, protect Maria, and head North.
Enter the Women’s Locker Room, pick up the ‘Shotgun’ from the locker, and find the ‘Bent Needle’ from the teddy bear on the table.
Cross the hall into the Men’s Locker Room and examine the bloody jacket twice to find the ‘Examination Room Key.’

Back to the 1st Floor

Return to the 1st floor and enter the Examination Room using your key.
Go through the only other door inside to the Doctor’s Lounge.
Pick up ‘Shotgun Shells’ in the sink, and check out the board on the left of the East door for a code.

Return to 2F

Go back to the 2nd floor and head through the big doors to the Southwest.
Pick up the ‘Handgun Bullets’ off the stools, and enter the first big double doors on your right.
Pick up the ‘First-Aid Kit’ and examine the typewriter for another code.
Leave the room, go into M2, and find ‘Shotgun Shells’ and ‘Lapis Eye Key’.
Enter room M3, defeat the ambushing monster, and pick up the ‘Health Drink’ and ‘Handgun Bullets.’
Enter M6, prepare for another ambush, and collect the ‘Shotgun Shells’ and ‘Health Drink.’

Move to the 3rd Floor

Backtrack to the stairs and take them to the third floor.
Kill the nurses, pick up the ‘First-Aid Kit’ at the Northeast corner of the hall.
Try the big double doors at the Southwest end. Use the Doctor’s Lounge code (7335 for Easy/Normal, 1328 for Hard).
Visit room S3 for a scene, then examine the desk to find the ‘Roof Key.’
Go to S11 to find a ‘Health Drink’ and a save point.

Access the Roof

Go back to the stairs, climb up to the roof, and use your key to go out onto the roof.
Approach the notebook and run down to the fence between the stairwell and the Control Room.
Watch the cutscene with Pyramid Head and regain control in a room with four North-wall doors.

Unlocking the Box

Enter the third door on the left, and examine the bloody mess for a series of numbers.
Go back to the Southwest double doors and enter room S14.
Use the Lapis Eye key and the Purple Bull Key to unlock the box.
Input the codes from the Memos in the button lock and cylinder lock. Retrieve the ‘Piece of Hair.’

Unlocking the Drain

Leave the room and enter the shower room next to the stairs.
Stand in front of the drain, combine the Piece of Hair and Bent Needle.
Retrieve the ‘Elevator Key’ using the combined items.

Using the Elevator

Save your game in room S11.
Use the Elevator Key and select the first floor.
Enter room C3, and find ‘Handgun Bullets’ on the floor and ‘Shotgun Shells’ on the end table.
Enter room C2 and approach the back for a scene. Your Brookhaven Hospital adventure concludes here.

Boss Fight Against the Hanging Demons

Equip the Shotgun as your primary weapon.
Position yourself in a corner. Aim the Shotgun to possibly hit more than one demon at once.
Unleash about two shots, then strafe to another corner of the room using the square button and either L1 or R1. Repeat this pattern.
Be aware of the demons’ attacks. They can either strangle you, which only occurs if you’re stationary (usually while reloading), or kick you, which happens very rarely.
Keep a close eye on your health to avoid getting defeated.
If you are grabbed for a strangle attack, rapidly press a set of non-menu buttons to escape quicker.
About ten Shotgun blasts from a decent range should kill the demons on normal difficulty.
Note that you start the battle against two hanging demons. When one is killed, another joins the battle. Distribute your damage evenly so you can kill the first two close together, and deal with the last one easily by itself. You will now find yourself waking up in an alternative reality hospital.

The codes to unlock the box are found in the memos. The “Imprint on carbon paper” provides one code that should be punched into the button lock on the right side of the box. The other memo, “Tern tern tern the numbers,” contains a code that should be input into the cylindrical lock (numbers facing forward from top to bottom). You also need to use the Lapis Eye key and the Purple Bull Key to unlock the box.
