The simple teleporter mod does exactly what you think it might do. It allows you to build a very simple teleporter which you can use to teleport to a player defined location on the map.
Once you have built a teleporter, you can pick a location on the map that will be the destination. You then link this location to a teleporter that you have placed on the ground. Then by standing on the teleporter and clicking shift, you will be teleported to the location on the map that you previously set. It is as simple as that!
In terms of performance, there are no specific overheads to using this mod. However, by instantly teleporting across the map, you are effectively loading the game world up again, without having a loading screen to buffer it. This often results in you having to wait a moment for all of the chunks to load up around you after initially teleporting.
The simple teleporter mod supports Minecraft version 1.11.2.
